  • Overview

    Responsible for professional nursing care and related assistance to patients/families with a focus on obstetrical and newborn patients. The Mother/Baby Care Coordinator is involved in direct and indirect patient care. This position provides direction to Licensed Vocational Nurses (LVNs) and Medical Assistants (MAs) as well as other non-licensed personnel in the provision of patient care. In collaboration with other health team members, follows the philosophy, policies and goals of LSCC.

    Responsibilities

    CARE COORDINATION:

     

    • Engage hard to reach patients through outreach and in person consultation. Plays a key role in a collaborative care model to address factors that affect maternal health and well-being: prenatal visits, postpartum visits, postnatal family planning, transition to primary care, and postpartum depression.
    • Collaborates with the Interdisciplinary Team in the planning and coordination of patient care
    • Coordinates and monitors care for the identified patient population through the first year post-delivery for mother and baby.
    • Facilitates newborn establishment and continuity of care with LSCC Pediatrics.
    • Create and lead health education initiatives for patients and families, focused on mothers and babies.
    • Facilitate transition of insurance coverage for new mothers after delivery
    • Develops and monitors mother/baby dyad registry, conducts outreach and facilitates scheduling.
    • Monitors process indicators for an identified patient population.
    • Identifies and facilitates access appropriate levels/types of care for an identified patient population

     

    ADDITIONAL CLINICAL RESPONSIBILITIES:

     

    • Performs patient assessments/reassessments including triage consistent with evidence-based practice for the RN scope of practice
    • Performs clinical and non-clinical skills proficiently in accordance with policies and procedures
    • Coordinates and participates in providing direct and indirect patient care

    OPERATIONS:

     

    • Works collaboratively with the clinic team to facilitate efficient workflow
    • Organizes work and utilizes time efficiently to meet programmatic and operational goals

     

    COMPLIANCE:

     

    • Prepare status reports as directed to update the status and implementation plans of programs, services, and quality initiatives
    • Participate in activities associated with performance improvement, quality initiatives, Meaningful Use, Joint Commission (TJC) and Patient Centered Medical Home.
    • Participate in monthly, quarterly, annual and ad hoc meetings as assigned
    • Performs to process and outcome metrics identified for key programs and initiatives

    Qualifications

    REQUIRED EXPERIENCE, EDUCATION, AND LICENSE: 

    • Graduate of accredited nursing school required
    • Valid Texas RN license; BLS certification from American Heart Association
    • Minimum of one (1) - two (2) years of nursing experience in obstetrics, gynecology, labor and delivery and/or pediatrics

     

    PREFERRED EXPERIENCE, EDUCATION, AND LICENSE:

    • Bachelor’s degree or Master’s degree in Nursing Science preferred
    • Previous experience in outpatient/clinic nursing preferred
    • Previous experience with electronic medical records preferred
    • Previous care coordination experience preferred
    • Previous project/program management experience preferred
    • Bilingual English/Spanish preferred
