    Interested in impacting patient care outside the clinic setting?  Here's your chance!  We are looking for a LVN to support indirect patient care by telephone and/or other electronic communications.   If you are customer service focused, enjoy being part of a collaborative team, and want to work in a fast-paced environment, please read on! 

    Responsibilities

    • Handling telephonic and/or other electronic communications and patient health education calls according to LSCC standards by using the navigation center contact center software
    • Provide individualized patient coaching using health education tools and the Electornic Health Record (EHR) system, focusing on training and encouraging patients into a state of wellness; identify patient personal strengths in an effort to recruit them to participate in improving and maintaining their health
    • Perform and ensure proper documentation of patient communications accurately in the use of the  EHR system
    • Interpret reports, data and medical information to determine the needs of the patient/family
    • Maintain patient care standards following established LSCC health education and information guidelines
    • Perform prescription refills and provides medication management teaching using established LSCC medication refill protocols and the EHR system
    • Complete Prior Authorizations/Referraks as necessary/when applicable
    • Perform clinical and non-clinical skills proficiently and in accordance with policies and procedures
    • Coordinate and participate in providing indirect patient care via telephonic and/or other electronic communications
    • Clinical interventions are implemented in a safe and appropriate manner and documented thoroughly
    • Perform care to meet the physical, psychological, psychosocial, emotional and social needs of the patients/families
    • Care is based on the patient’s age, developmental stage, educational needs and family/guardian expectations
    • Collaborate with the Interdisciplinary Team in the planning of patient care
    • Document patient interactions in EHR
    • Schedule appointments as appropriate in EHR
    • Communicate barriers and patient health needs to appropriate nurse and/or provider
    • Contact the appropriate health care provider of patient/family concerns
    • In collaboration with other members of the health team provides health care teaching to patient/family or significant other (in compliance with HIPAA Standards)
    • Provide for patient safety at all times
    • Participate with in data collection, focus groups, TJC, PCMH, and other quality improvement initiatives
    • Organize work and utilize time efficiently based on knowledge of nursing theories and proceduresAssume responsibility for his/her own professional development and for sharing knowledge with others
    • Invest own time and effort to improve knowledge, skills and judgment
    • Responsible for knowledge and compliance of all LSCC policies and procedures
    • Maintain professional manner and appearance
    • Provide and promote customer service
    • Other duties as assigned

    Qualifications

    • Graduate of accredited nursing school is required
    • Valid Texas LVN license is required
    • BLS certification from the American Heart Association is required
    • Previous experience in outpatient/clinic nursing is preferred
    • Previous work using Health Medical Records is preferred
    • Bi-lingual (English/Spanish) is highly preferred
